Image Generation Tool Error: Persistent Issues with GPT-4o & 4.1

Hello 👋 Is image generation broken? I created a new agent (tested on gpt-4o and 4.1) with image generation tool and I always get the same error: "I encountered an error while trying to generate the illustration for you. Could you please try again later or let me know if there's anything else you'd like to do?" Thanks
